During your stay, your doctor will advise when you can expect to be discharged home. To ensure your well-being, we advise that a family member or friend be with you on the day of your discharge. If you think you will need support at home, please talk to the staff caring for you so that the required support can be arranged ahead of time.
Before you leave, our Nurse/Patient Service Assistant will assist you with the following:
- Arrange for follow-up appointment at the Specialist Outpatient Clinic (SOC), if necessary. Should you need to reschedule your outpatient appointment after your discharge, please call your Specialist Outpatient Appointment Line or fill up the online Appointment form.
- Bring you the medicine that your doctor has prescribed and advise you on the medicine's usage.
Please check your bedside cabinet and wardrobe to ensure that you have not left any personal items behind.
Please remember to withdraw any personal items that you may have placed in the safe, or with our Patient Service Centre.
Please return all hospital gowns, children's pyjamas, baby vests and blankets.
Do remember to follow the instructions given to you on your medication and mark your diary for the follow-up appointment with your doctor.
Discharge from the hospital is 12.00 noon. An additional day's rate is applicable after 12.00 noon. Patients are responsible for making their own transport arrangements to and from the hospital. Please ensure you have collected all your personal items. Before leaving the ward, please make sure you collect any private x-rays or medications and confirm if you have any follow-up appointments. If you are waiting for a relative or friend to accompany you home, you may wait in the ward's Day Room.
